Community Nursing


Department of Veterans' Affairs (DVA)


We proudly support veterans and their families by providing high-quality, personalised healthcare services under the Department of Veterans’ Affairs (DVA) arrangements. Our goal is to ensure that veterans receive the care, respect, and assistance they deserve.


Our in-home and community-based healthcare services are designed to enhance independence, wellbeing, and quality of life for DVA cardholders. We offer personal care, domestic assistance, nursing services, and social support.


DVA Gold and White Cardholders may be eligible for these services based on their specific needs and entitlements.

Support for Children & Young People


We provide specialised care to ensure the safety, wellbeing, and personal growth of children and young people. Our person-centered, trauma-informed care approach focuses on creating a stable, nurturing environment where they can develop confidence, independence, and essential life skills.


We tailor our support to meet individual needs, helping young people engage with their community, build meaningful relationships, and work towards their goals. Through education, social interaction, and personalised assistance, we empower them to navigate life’s challenges and reach their full potential.


We also partner with Safe Living Australia to further support vulnerable children and young people, particularly those with urgent and immediate care needs.


  • Better Care, Better Services

    We are committed to striving for excellence in the standard of safety and quality responses for children and young people. The Children and Community Services Act 2004 (the Act) provides the legislative framework for the development of standards for children and young people in care, or where there may be concerns regarding a child’s safety. We adhere to the Better Care, Better Services framework and quality standards below.


    Quality standards:

    1. Children and young people are provided with, and experience, stability and consistency during their time in care.
    2. Children and young people, and those important to them, are continually engaged to participate in planning and decision-making that impacts on their lives and their future.
    3. Aboriginal children and young people are supported to maintain meaningful connections to their family, community, land, and culture in accordance with the Aboriginal and Torres Strait Islander Child Placement Principle.
    4. Children and young people’s needs are met through individualised assessment and child focused practices, encompassing all aspects of their lives and wellbeing.
    5. Children and young people are supported to develop their identity and a sense of belonging through their families, friends, culture, spiritual sources, and communities.
    6. Children and young people are adequately prepared and provided with timely and high-quality support, as they transition out of care and into adulthood.
    7. Children and young people are supported and empowered to know their rights, raise their concerns, and have these responded to and resolved in a timely manner.
    8. Children and young people are provided high quality and safe care by well trained and supported staff and carers.
    9. Organisations are child focused and accountable.

COSA


Continuity of Support Arrangements


We understand that individuals who are ineligible for the NDIS may still require assistance. Our COSA program ensures that those previously receiving state or territory disability services can continue to access essential care.


We tailor our support to individual needs, focusing on maintaining independence, wellbeing, and community participation. Our services include personal care, daily living assistance, and social engagement, ensuring a seamless transition and continued access to quality care.


Supported Independent Living (SIL) for people with Dementia


Our personalised dementia care supports individuals in maintaining dignity and a high quality of life with as much independence as possible. Our Support Workers are trained to offer compassionate assistance, creating a safe and familiar environment whilst promoting physical and emotional wellbeing.


Accomodation support can be provided for individuals with high care needs requiring 24/7 support, shared living arrangements, or one-to-one support. Whatever you need, we ensure peace of mind for loved ones while empowering those with dementia to live comfortable and reduce risks of harm. Each individual is respected and nurtured, maintains their dignity, and is safe from abuse and neglect.
