Supported Independent Living (SIL)

Accommodation support can be provided for individuals with high care needs requiring one-to-one 24/7 support, through to shared-living arrangements. Whatever you need, our team finds the perfect home and matches you with a dedicated support team tailored to your unique needs.

Supported Independent Living (SIL) for people with Dementia
Our personalised dementia care supports individuals in maintaining dignity and a high quality of life with as much independence as possible. Our Support Workers are trained to offer compassionate assistance, creating a safe and familiar environment whilst promoting physical and emotional wellbeing.
Accomodation support can be provided for individuals with high care needs requiring 24/7 support, shared living arrangements, or one-to-one support. Whatever you need, we ensure peace of mind for loved ones while empowering those with dementia to live comfortable and reduce risks of harm. Each individual is respected and nurtured, maintains their dignity, and is safe from abuse and neglect.
In-Home & Social Supports
Whether in your own home or in the community, Plan and Grow's In-Home and Social Services Division can provide you with supports ranging from assistance with daily living and personal care supports to social and community participation.
Our team of highly trained and skilled support workers are here to assist.
Short Term Accommodation
Short-term Accommodation stays with Plan and Grow offer a well-deserved break for both participants and their families or carers. It’s a great way to recharge and step away from the routine, in a relaxing environment. We also provide stays for emergencies and respite.
Our homes are fully furnished, clean and ready for you to put your feet up. Plus, your NDIS plan will cover support staff costs, accommodation costs, and all your meals during your stay. You can get lost in books, play board games, or enjoy your favourite show on the smart TV.
Support Coordination.
Navigating your NDIS plan can be complex, but we're here to help. Our experienced support coordinators will work with you to understand your plan — whether you're new to the process or already familiar with it — and connect you with the right services, tailored to your needs.
We focus on building your capacity by assessing your current supports, linking you with the best allied health professionals, and guiding you with your allocated funding.
