Our Community Nursing (CN) services, provided under the Department of Veterans' Affairs DVA arrangements, offer in-home care to veterans, delivered by skilled Registered Nurses, Enrolled Nurses, and Personal Care Workers (PCWs).

Our mission is to help veterans maintain their independence and avoid early hospital admission.

Plan and Grow's Community Nursing (CN) Program provides essential nursing and personal care services at home including:


  • Help administering medication
  • Wound care
  • Hygiene, and dressing
  • Palliative care options


These services can be provided face-to-face or remotely (by phone/online) when appropriate.


Before delivering services, a risk assessment of the home environment is conducted to ensure safety.

Accessing Community Nursing Services


To receive Community Nursing (CN) services from Plan and Grow, a written referral is needed from your GP, hospital, or specialist nurse.

Referrals are valid for 12 months (from GPs) or 6 weeks (from hospitals).

A new referral is required if you've been out of care for more than 28 days or decide to change providers.

The referral should be sent directly to a DVA-approved provider (Like us!) where a registered nurse will assess your needs and prepare a care plan.

These services are fully funded by DVA for eligible individuals, provided by approved CN providers.

You may be eligible to receive community nursing services and/or personal care at home if you hold a:
  • Veteran Gold card
  • Veteran White Card holder with a condition eligible for treatment
