High Care Needs Out of Home Care
Plan and Grow are one of Western Australia’s most trusted complex care needs providers. Our Out of Home Care service offers a safe, nurturing environment for children and young people with high care needs.
Child-Centered & Needs-Based
Care plans are centered around each child’s specific needs, with flexible options for 1:1 staffing when necessary to ensure safety and attentive care. Only competent staff, with extensive training in complex care, are placed with each child. Our team is equipped to provide the highest quality of care and support.
Children may be suitable to share with others only when thorough risk and matching assessments are undertaken to determine safety considerations of a shared model.
Aboriginal and Torres Strait Islander Care Model
With guidance from elders and Aboriginal care teams, we offer a care model that emphasises connection to culture and community, providing children with consistent cultural support and identity reinforcement.
Two Hour Fast Intake
When immediate care is required, Plan and Grow apply our Fast Intake service which promises to provide a home and care team within two hours of the request to provide care.

At Plan and Grow, our primary focus is to provide children and young people with a supportive environment and the necessary tools to empower them to succeed in their own endeavours. We are a team driven by our passion to offer a safe haven and sanctuary for children.
Through our trauma-informed practice plan, we aim to create an atmosphere that acknowledges and addresses the impact of trauma on children's lives. We understand that many children have experienced adverse events or challenging circumstances that can significantly affect their well-being and development.
By adopting a trauma-informed approach, we ensure that our team members possess the knowledge and skills to recognise the signs of trauma and respond compassionately. We create an environment where children feel understood, validated, and supported in their journey towards healing and growth.
